I think the fact that any kind of exertion can cause/trigger PEM strongly suggests that whatever the underlying primary problem is it must be something that has a common element throughout the entire body and its functions (or almost all of it).
Doesn't even need to be overhead, though that is the worst position. Holding them horizontal is also a problem. Any activity that requires arms being (actively) held up against gravity to any degree is a problem.
You might be thinking of the article from the Royal Australian College of General Practitioners (RACGP) on 'incremental exercise' for ME/CFS, written by Glasziou, which only cited PACE, Larun Cochrane (2019 version), and for some opaque reason an article on Long Covid research.
The actual Oz...
CBT is the most studied treatment, with promising effects for patients with, for example, chronic fatigue syndrome,
After 35 years of dominance and endless rigged studies, and they are still stuck at the merely 'promising' stage?
How is this not widely dismissed as a complete failure of this...
Worth repeating.
The real situation is so bad and deeply entrenched that is difficult to get those unfamiliar, and even some familiar, with it to understand and believe it really is that bad. Nobody wants to face up to the truth that things really can go that far off the rails, inflict that...
That is my reading of it.
the paediatricians in the current study commonly reported the engagement of other disciplines, particularly psychologists and physiotherapists,
Interestingly, most paediatricians reported recommending GET as a common part of their practice. Despite the demonstrated...
No doubt the psychosomatic club will spin any outcome to suit their purposes.
But the rest of the world might not be as accepting of that fairy tale as they once were if a biological explanation is found, and especially if a related treatment is indisputably producing genuine cures, or major...
Fair question.
In fairness, it has the potential to cut the other way. If a biological mechanism is found for ME/CFS, and it correlates strongly with scores on 'somatisation' check lists, and especially if both resolve with the same biological treatment, then that is very good evidence for...
Which could be interpreted as non-pathological compensatory or displacement movements. (Not sure of the correct terminology.)
I occasionally get a slight rhythmic side-to-side shaking of the head when tired or dealing with a high level of PEM (like today, after an hour in the dentists chair...
To judge the appropriateness of distress requires a God-like view of the human experience and condition, which no individual or group can ever possibly have.
It is an absurd and reckless claim in the first instance, and every other instance.
Exactly.
If one was looking for 'perpetuating psycho-social factors' in conditions like ME/CFS, this iatrogenic crap and its consequences (including for accessing material support) is among the most potent of them all. And the most unnecessary.
Problem, of course, is that it reflects very...
The blindingly obvious problem being, of course, that this kind of list of 'symptoms' is exactly what you would expect to see in a child who has a serious physical problem that is undiagnosed, and being misdiagnosed and mistreated as a mental health problem.
The appropriate safety factor, the...
