There are an awful lot of references to "recommendation: strong, quality of evidence: low" in that paper, as well as quite a few references to "recommendation: strong, quality of evidence: weak".
I am sure there is some logic to describing recommendations as strong while describing the quality...
I suspect one factor in this situation is that the people most likely to apply to study medicine must believe beforehand that they are healthy enough to do the training, which is well known to be physically and mentally demanding. Any medical students not up to the job will drop out or be kicked...
