My personal response to this questionnaire is that it makes me feel sick too. Patronising, lacking in awareness/insight/empathy. As mentioned above by Adrian, would make more sense if administered after recovery rather than while ill, if at all. But to someone who is terribly chronically ill...

Reading that document, it looks like Esther fails pretty much everything under Guideline 4: Only Good Science. So then I wondered what one could do as respects notifying TEDx in such a case. Reading further, there was this paragraph at the end of Guideline 4:

As well as the self-delusional aspects of her talk, there is also her speaker bio page at the TEDx website which claims that Esther's "research is world leading." https://tedxbristol.com/about/person/esther_crawley . I take it that TEDx doesn't verify what their speakers claim about themselves...
Thank you! It wouldn't play in my browser (Firefox), got a Format Not Supported error message, but I downloaded it with the Firefox add-on Video Download Helper, and it's actually the full video, not just an audio recording! It's pixellated, but watchable enough to see Esther's performance...

Someone please correct me if wrong, but from what I've seen, what she's claiming was an email to herself was actually a mock-up of a phone call to Simon Wessely and was created by an artist at the Sunday Times:
