    Been wearing a Garmin with HRV monitoring for a couple of years. My HRV reliably drops when I've had a stressful day or consume alcohol. Haven't noticed any correlation to crashes. N=1 though.

    This looks like a reasonable mechanistic explanation for exertional intolerance. Orthostatic hypotension as well. Not sure how it would explain PEM unless that is through the sodium and calcium overload.

    Fludrocortisone

    I have mild symptoms overall. I have been on fludrocortisone for many years and it definitely helps my OH. I usually don't have to worry about standing up too fast unless I'm in a PEM phase or in a very hot environment.
