Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

Opposing MEGA point out on their Facebook page that the Bedfordshire Chronic Fatigue Service appear to already be seeking to subvert the new NICE guidelines by offering CBT and GAM (graded activity management) as treatments. Obviously it needs clarifying if they include ME/CFS within ‘people...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

As @Lucibee says the Pulse article referring to the Guidelines as released in response to an FOI request links to what looks like a pdf of the still embargoed 4th of August final draft, shared by NICE to all steak holders signing a confidentiality agreement. It is stored as a pdf on an ME...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

But wasn’t this an FOI request submitted to and replied to by the DHSC, rather than NICE?

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

And will the relevant Royal Colleges now feel they have been used and abused by the PACE advocates, set up to look foolish? Who will the SMC now go to for ‘expert opinion’ on publication of the new guidelines? What will follow the article to appear in the Journal of Internal Medicine (see...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

Presumably the Guidance Executive at NICE are meeting today, as they seem to usually meet on Tuesdays, so presumably we will hear tomorrow their timescale for publishing the new guidelines if they follow a pattern of a next day online announcement. We don’t know if the proposed ‘clarification’...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

Also once the new guidelines are published we can respond to such with the statement that they are acting against the conclusions of their own professional bodies, ie the medical Royal Colleges, whose representatives have agreed to support the new guidelines and that these interventions are...

Metacognitive Performance on Memory and Visuospatial Tasks in Functional Cognitive Disorder, 2021, Pennington et al

[Sorry This is too long and confused, but I am not with it enough today to rewrite it in a more easily understood form.] There is enormous variation between people with dementia as to how aware they are of their problems. One difference I would use as a possible indicator in differential...

Metacognitive Performance on Memory and Visuospatial Tasks in Functional Cognitive Disorder, 2021, Pennington et al

To be fair there are many other situations where people with cognitive impairments are very aware of their own deficits, and it is not uncommon for people with dementias, at least in the early stages to have insight into their problems.

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

There is scope here in relation to supporting people with chronic illness to tailor that support to the specific issues the individual is experiencing, especially as we have no clear evidence supporting any one particular approach. I had big issues changing my behaviour to do less, to...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

Thank you for clarifying that. So @MSEsperanza was correct in suggesting that the ‘pull out’ quote on the NICE page is where the change happened. On the 12.22 snapshot ‘publication next week’ was clearly stated, though there was a discrepancy between the pulled out quote and the final sentence...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

NICE did say “We are now confident that the guideline can be effectively implemented across the system and we will therefore publish the guideline, with additional clarification where necessary, next week" on line in the material put out by last Wednesday. I initially missed it and I was puzzled...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

A great article, all of which is eminently quotable, but here is a bit I selected to highlight: I do wish we had video clips of the relevant BPS ‘great and the good’ responding.

[Blog] BACME, NHS ME/CFS clinics shift from deconditioning to dysregulation model of ME/CFS in anticipation of updated NICE Guideline

Perhaps a red herring, but I wonder if there are at least two potential components in unrefreshing sleep. I regularly wake up in the morning feeling terrible and need several hours rest before I can face the world. Often I will get up for a short will, to bring the milk in, feed the cat when...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

The NICE announcement did previously include a clear statement that the guidelines would be published this week in addition to the ambiguous ‘after the meeting’ phrase, but I can’t find it any more, so I wonder if the final sentence @Trish quotes has been edited during the week: [sorry cross...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

I thought this to, but it was pointed out to me that separately to the reference of ‘after the meeting’, NICE also say the guidelines will be published next week. So presumably this means some time between the end of the Tuesday? meeting and the end of the day on Friday 29th. Still the wording...

Publication of the NICE ME/CFS guideline after the pause (comment starting from the announcement of 20 October 2021)

This Pulse follow up article clearly indicates the rejection GET and ends with juxtaposing mention of PEM with Long Covid It is useful getting this out to GPs. Note, It seems sometimes I can access the article and then it is refusing access unless registered.

[Blog] BACME, NHS ME/CFS clinics shift from deconditioning to dysregulation model of ME/CFS in anticipation of updated NICE Guideline

In theory I see nothing wrong with clinics that see a variety of patients, the problems arise when they assume their caseload are a homogenous grouping and respond with a one size fits all approach. I suppose we should be asking, how is it that the current ME/CFS clinics are unable to...

[Blog] BACME, NHS ME/CFS clinics shift from deconditioning to dysregulation model of ME/CFS in anticipation of updated NICE Guideline

I sometimes wonder if this is how a lot of science works. You need stages of synthesis, which more often than not requires people to be sufficiently over confident to assert things on inadequate knowledge, this then set the tone for the next few decades of research proving just how wrong they...

Functional Neurological Disorders - discussion thread

One wonders how many people would be able to fake a serious mental illness that is such that it seriously disrupts their daily life and makes it impossible to work. Also why would most people want to fake such an illness. I suspect the cost of such as severe depression or schizophrenia is far...
