Yes, I can understand that upsetting a doctor may be counter productive and backfire. But I don't see the point of avoiding to tell the blunt truth to protect any quack's feeling .
So telling. It's all about the doctor, their life, their stress, the heartsink patient the poor physician has to deal with. The "functional" patient is just another annoyance that they need to get rid of, with an appearance of humanity to let the doctor think they are caring.
Is knowing what mediates effects of a treatment that "may be helpful"* according to the abstract of the study helpful?
*and not effective if you look a bit closer according to Tuller and Tack
