The poor man is trying to explain to himself what he ought to be thinking. Perhaps he should realise that no explanation of this sort is needed. cytokines make you feel yuk - that is all we need to know.

No it is a different substance altogether. One is alpha and one is beta.

    Concerns about Cochrane

NICE gathers all relevant publications but that is different from a meta-analysis. Inmost cases you look at the publications and if there is solid evidence from any of them that is enough. Most drug regulatory authorities require more than one study showing efficacy but NICE has on occasion...

If I remember rightly you feel terrible with hours of the interferon injection. It is likely that you have had the hepatitis for years at that point.

    Concerns about Cochrane

I think NICE have their own reviewing procedure. That probably includes picking up what Cochrane has done. I have no heard of NICE getting reviews done by others but it is possible.

It is based on things he has said in the past and in particular on a referee's report he wrote on someone else's paper that became public. He basically said that any paper that did not say that ME was biopsychosocial had to be rewritten (he considered that non-negotiable). He might have changed...

I agree that not very much insight seems to have emerged from the studies themselves relating to mechanism. The interferon does not hang around long. I do not think there is any doubt that it is the interferon rather than the virus that causes the fatigue or that the fatigue goes on far longer...

I agree. I think there is avery important message in the alpha interferon story - that fatigue can persist long after the triggering signal has gone. Some other signal must then take over. Moreover, there is no need for any specific autoimmune response. The pity is that Pariente is still as far...

So who tweets for AfME and is happy to quote 'classic psychosocial factors' as if it meant anything?

    Concerns about Cochrane

I think that broader perspective is very useful. Basically, meta-analyses were never very powerful. They are now probably completely misused and useless. If you think about it, if individual studies are good enough there should be no need for any meta-analysis. And in the great majority of...

Simon Wessely: ‘ECT is in my own advance directive’

But the point is that it is used for people who are already live zombies, like my wife. I am glad to have her back alive and well. Your argument seems to me to be based on emotion and hearsay rather than what is good for people. The stuff about burning out bran is nonsense. The ECT practitioners...

    Concerns about Cochrane

When I did a review in about 1995 people just volunteered to do a review and submitted it for approval. They were not necessarily paid. However, junior academics would get credit for doing reviews. More recently I think the centres have regular incoming funds and put staff members on to reviews...

UK CMRC 2018 Conference held September 19 & 20 at Bristol

It doesn't really matter where. Probably the best idea would be to gather together a few like minded people and then see if more like minded people want to join in. The landscape has changed at CMRC sufficiently for it to be possible to do that without appearing to be in conflict with CMRC...

UK CMRC 2018 Conference held September 19 & 20 at Bristol

Yes, it is a pity twitter oversimplifies so much but some important points made. As Simon says, if subjective reports and objective measures do not match it does not necessarily mean the objective measure is the wrong one. This again emphasises to me the importance of having a measure that...

    Concerns about Cochrane

It may well be that members do not want to engage in anything that threatens to upset any apple carts the way Gotzsche likes to. I also wonder if this was a secret ballot or a show of hands. It is interesting to ponder why people do Cochrane reviews. I am pretty sure it used to be mostly a...

    Concerns about Cochrane

My arithmetic suggests that the Board had at most 8 elected members, including Gotzche. Of these one was sacked and four out of a maximum of seven others resigned in sympathy. It might have been four out of six. That does not make Gotzsche sound like a heretic outsider. The majority of elected...

Bristol - New Network - Prof Munafo

I have left a comment much to this effect on Munafo's page on Wonkhe about the manifesto. It is not clear that these comments will be posted up.In fact i doubt they would want to post mine, since it is in a sense a personal message.

The piece by Munafo and Bishop et al is all very well but I think it could be described as 'fluff-wiping'. (The practice of starting to do a job by picking something up, wiping the fluff off and then putting it down and forgetting to do the job.) Pretty much everything in the manifesto is...
