    The "training effect" is becoming more comfortable with the testing procedure and your level of performance. You don't expect to do it perfect first time right? It is the VO2peak that is often improved slightly (1-2%), the ventilatory threshold etc. is normally unchanged.

    What you are suggesting is a double edged sword - the presentation and recording of the symptoms are not the symptoms themselves and may reflect those same biases you mention, while the underlying illnesses were genuine.

    I agree this would be an interesting question, along with measuring OI specific symptoms post exertion. Personally, I only experience OI after substantial exertion (for me), but it occurs sooner than the rest of the post exertional exacerbation.
