The sodium observation is very odd and I suspect reflected change in compartment size rather than any membrane abnormality. I am not aware of evidence relating to 'impaired recovery'? The metabolic differences after PEM from Hanson were mostly of less change than normals I thought. None of this seems to me to tell us much. people have been obsessed with there being a muscle problem since the 1980s but the evidence has been pretty pervasively negative and it does not make a lot of clincial sense.

The question is whether muscle pathology is a cause, consequence, or part of a wider systemic feedback loop.
Not if we cannot find any muscle pathology. Competent professional pathologists looked in the 1980s (one of them is someone I know very well, I worked on the same corridor). Some recent studies have suggested pathology but they are hard to square with what we know from previous work.

Unfortunately, the sample size is pretty small, as is often the case.

Or this one: the study found significant changes in blood metabolites following exercise:

I vaguely remember there was another study where, during a second exercise test on the following day, performance or VO2 max declined much more than in healthy controls.

I’d have to look through the discussions here on the forum about those two studies to remember whether there were any major methodological issues with them.

But I do think this is an avenue that’s worth investigating more closely.

These findings increasingly converge with research on

#MECFS and #LongCOVID, where pathological vascular changes in skeletal muscle have also been demonstrated, including recent work from the group of Rob Wüst (Slaghekke et al.), presented at the ISLC-PAIS conference in Amsterdam in August 2026. The scientific approach of #Mitodicure could therefore also show promise for Fibromyalgia.
