The symptom signaling theory of ME/CFS involving neurons and their synapses

How does this hypothesis account for the waxing and waning of symptoms in severity level? As in people going from mild to severe, back to mild, repeat.

I remember some discussion of autoimmune diseases following a similar cycle, with periods of increased symptoms.
