I've not been impressed by the evidence that painkillers improve outcomes over the long-term for chronic pain, but I also think it's important that it's patients themselves who get to decide what is best for them.
I've got some concern that NICE mainly seem to want treatments that cannot be assessed under blinded conditions.

The first-ever NICE guideline on chronic pain has said that GPs should not prescribe opioids to patients with chronic pain because they could be ‘harmful’.
Instead, GPs should consider alternatives such as certain antidepressants, an exercise programme, CBT or acupuncture.
Published today, the long-awaited draft guidance says GPs should avoid prescribing commonly used medicines including opioids and gabapentinoids because there was limited evidence of their effectiveness in managing chronic pain.

12social factors predict successful pain management. The committee acknowledged
13the importance of a comprehensive biopsychosocial approach to assessment and
14management. They agreed that it is important for the healthcare professional to
15understand how pain is affecting a person’s life. A care plan should be based on the
16effects of pain on day-to-day activities, while acknowledging that it is not possible to
17predict what might happen in the future.
