I was about to say something like this. I suspect this could be a reason that men with ME get treated so badly. How dare these men let down men in general by succumbing to a women’s hysterical condition!! They are perhaps seen as no better than women, and treated as such.
The historical...
I think tests are only discouraged after disgnosis. Initial tests to rule out other possibilities are recommended. But if those tests don’t pick the problem up, it seems they should be regarded as definitive*, and further explorations are discouraged and CBT/GET is the next step.
* IE They...
Yes - and this type of fatigue research should not be described as ME research. It should not suck up funding that could otherwise be used on ME specific research!
I’ve been dipping into this debate occasionally. I hate the very thought of this type of treatment, BUT I have also seen it be highly effective on an individual well known to me.
This person had a complete turn around in their life after treatment. And now 20+ years later, is still leading...

So this feasibility study game seems to be a blatant attempt to have the full trial (that the feasibility study gets rolled into) labelled as “prospectively registered”, instead of it carrying a more accurate descriptor of “retrospectively registered”.

I don’t actually think statisticians are generally to blame for this. Although it would be good if they all insisted on caution being applied to outcomes derived from subjective scales.
Assigning scale numbers to subjective responses, is the real problem as you rightly point out.
Doing this...
I think also we want some of those folk who have been applying GET under the direction of their seniors, to feel that they can pause and take a review of their outcomes. There must be quite a few who are watching and listening to the debates around PACE and who will eventually feel they can...
Here’s a thought. Bear with me. . .
My background is biology, and as a student, when surveying plants in the wild, we always used quadrats that were placed in very precise predetermined locations. So if surveying a transition from wood to grass land for example the quadrat positions could...
Exactly @andypants and this could also be a reason GET seems to work.
Therapist hat on:
Take several weeks to “establish a baseline”, and ensure it is well below patient’s current activity levels.
Then oh so gradually increase in a graded manner. (If chosen baseline is really well below...
Totally.
I can succumb either from a single over-exertion, or from a slow accumulation of little extra amounts of exertion. The latter is the most difficult to predict as the accumulation can be over a period of a couple of weeks..
